In general, the final transformational phase results in a sustained ‘new ways of working’, based on in particular new pathways, use of telemedicine and continuous sharing of data between disciplines. However, based on extensive experience with large-scale regional transformational projects, HIM SL strongly holds to the concept that transformation is a continuous dynamic process. The preceding phases of evaluation, planning, implementation and re-evaluation not only take several years, but require a maximal flexible culture among and between the various care professionals and their management. Hence, HIM SL considers a culture of continuous improvement as the sum of optimising the availability of real time data and its use in an agile manner within an interdisciplinary team of formal and informal members.
